Aerial includes:

Care Coordination

Aerial™ is designed specifically for coordination and connectivity between interdisciplinary care teams, with community-based EHR, clinical decision support and a patient engagement toolset, along with robust data integration capabilities.

The Aerial Case Management module is a fully configurable, rules-based, automated workflow solution that applies accredited clinical content to the development of the most appropriate interventions and plans of care for each individual member or patient.

The Aerial module improves care management by:

  • Identifying members who would benefit from care management programs, consolidating medical information and case management data, and providing members with access to health and wellness material.
  • Delivering care managers the insight needed to develop the most efficient and effective intervention, including a patient-centered care management hub, which provides a complete clinical picture.
  • Allowing care managers to conduct a screening assessment, using access to summaries of prior medical, pharmacy and laboratory services to determine if the patient is eligible and amenable to management services.
  • Helping members better communicate with providers and learn better ways to manage their illnesses.
  • Enabling the Aerial Disease Management module to support personalized interventions to help you meet NCQA and HEDIS requirements, with seamless co-morbidity management and counseling from registered nurses, and regular communication with a member’s physicians.

Advanced Clinical Content

When a patient agrees to participate in case management or opts to enroll in a disease management program, the care manager follows up to assess ongoing healthcare needs, and using NCQA-certified and URAC-accredited Aerial™ Clinical Programs, develops a plan of care.

Aerial Clinical Programs use extremely sophisticated questionnaires with built-in logic, and evidence-based medical standards to enable care managers and members to collaborate on the most appropriate care decisions. Each response during an assessment triggers the production of a “problem list.” Working together, the care manager, member/patient and provider identify which of the potential issues identified during the assessment are of the highest priority.

These potential problems are addressed through a personalized plan of care unique to each member’s specific condition and long- and short-term goals.
